Pricing
New patient visit - $425 (extended appointment including chart prep and record review prior to visit) Currently there is a waiting list so please email the office to be added to our list. It will be important to complete and return intake form prior to visit once scheduled.
Established patients: After your initial visit, most people will require a follow-up appointment approximately 4-6 weeks to review labs (if ordered), and to further refine our treatment direction. Patients must be seen at least once yearly to maintain an active chart at Vital Roots.
15 minute phone consultation $45
30 minute appointment $90
60 minute appointment $180 (typical first follow-up with lab review)
Transfer from Mountain Medical Arts:
Patients previously established with me at Mountain Medical Arts will require an appointment to create a chart at Vital Roots Natural Medicine before any medical services can be provided. This first visit will be $180 if it has been within 3 years of being my patient, otherwise it will be scheduled as a new patient visit.
Packages
Discounts are offered for prepaid for future visits. (Prepaid appointments can be used in 15 minute appointment increments as needed)
New Patient Consult package -Initial intake appointment and also 120 minutes of followup appointments: normally $785, pre-purchase for $700 - Save $85
Prepay for 3 hour-long appointments (180 minutes) : normally $540 Save $50 by pre-purchase for $490
